PENGLIHATAN GANDA (DIPLOPIA) Jika anda melihat satu benda tampak sebagai dua benda, benda, anda mungkin mungkin menderita Penglihatan Ganda (Diplopia).
Apa itu Diplopia? Diplopia adalah gejala dimana pasien melihat dua tampilan dari satu objek. Hal ini dapat terjadi ketika satu mata ditutup (diplopia monokuler), atau hanya ketika kedua mata terbuka (diplopia binokuler). Pada diplopia monokuler, kemungkinan kemungkinan penyebabnya adalah kesalahan refraksi tidak t idak terkoreksi, gangguan kornea, katarak dan gangguan retina. Diplopia binokuler terjadi karena ketidaksejajaran mata, yang mungkin disebabkan oleh oleh gangguan pada 1) saraf 2) otot 3) persimpangan otot saraf 4) tulang sekitar mata.
Diplopia Binokuler – Masalah Masalah Saraf Pergerakan mata normal terjadi ketika otot-otot yang menempel pada mata bekerja dengan baik dan saraf-saraf yang merangsang otot-otot ini bekerja secara normal. Saraf-saraf ini berasal dari otak, dan jika satu dari saraf-saraf ini bermasalah, mata tidak dapat bergerak dengan sempurna kearah tertentu, mengakibatkan penglihatan ganda karena ketidaksejajaran kedua mata. Beberapa penyebabnya dapat membahayakan jiwa, sebagai contoh: aneurisme dan tumor otak. Pasien juga dapat mengalami kelopak mata turun pada mata yang bermasalah tersebut, juga posisi mata tidak normal dan pupil yang membesar pada sisi mata yang bermasalah. Jika disertai sakit kepala atau leher kaku, sangatlah penting untuk mencari bantuan medis secepatnya. Salah satu penyebab penting lainnya adalah tumor yang berasal dari belakang hidung (kanker nasopharing). Masalah saraf juga umumnya terkait dengan penyakit diabetes, hipertensi, hiperlipidemia dan kebiasaan merokok. Pada umumnya masalah saraf akibat penyakit penyakit ini hanya sementara, banyak yang sembuh dalam waktu 4-6 bulan. Kasus kasus kasus ini seolah olah seperti ‘strok kecil’ pada saraf. Tidak ada pengobatan tertentu untuk penglihatan ganda yang disebabkan oleh penyakit-penyakit ini. Meskipun, keberadaan penglihatan ganda mengingatkan pasien untuk mengontrol penyakit-penyakit tersebut. Penyebab lainnya dapat berupa macam-macam peradangan dan infeksi.
Diplopia Binokuler – Masalah Masalah Otot Penyakit-penyakit yang mempengaruhi otot yang menggerakkan mata dapat mengakibatkan penglihatan ganda. Salah satunya adalah penyakit penyakit tiroid mata, yang menyebabkan otot mata
membersar dan kaku sehingga tidak dapat bekerja dengan normal. Gejala lain dari pen yakit tiroid mata termasuk keringat berlebihan dan kehilangan berat badan meskipun nafsu makan meningkat, jantung berdebar, kecemasan, tangan gemetar dan satu atau kedua mata menonjol secara bertahap. Mungkin juga terlihat benjolan di leher bagian depan (meskipun penyakit tiroid mata dapat terjadi tanpa adanya gejala benjolan di leher). Penyakit otot keturunan (miopati) mencakup kondisi-kondisi berbeda yang mempengaruhi berbagai kelompok otot dalam tubuh. Banyak jenis miopati ini mempengaruhi otot mata juga. Biopsi otot dan/atau tes darah dapat membantu mengkonfirmasi diagnosa.
Diplopia Binokuler – Penyakit Persimpangan Otot- Saraf Miasthenia gravis adalah gangguan sistem kekebalan tubuh dimana antibody menyerang persimpangan otot- saraf dari kelompok otot yang berbeda-beda dalam tubuh. Jika otot mata terkena, pasien dapat mengalami kelopak mata turun dan/atau penglihatan ganda. Karakteristik penyakit ini adalah gejala biasanya membaik saat bangun tidur atau setelah beristirahat, dan memburuk sepanjang hari setelahnya. Miasthenia juga dapat mempengaruhi bagian tubuh lainnya, dan gejala seperti sesak nafas, sulit menelan, suara melemah, otot tangan kaki melemah mungkin merupakan indikasi miasthenia keseluruhan. Miasthenia parah dapat membahayakan jiwa, misalnya pada saat bernafas menjadi semakin sulit dan masalah menelan mungkin mengakibatkan tersedak ketika makan, infeksi paru dan kesulitan bernafas.
Diplopia Binokuler – Masalah Pertulangan Mata terletak pada kompartemen tulang tengkorak yang disebut orbit. Cidera yang mengakibatkan keretakan tulang orbit dapat menyebabkan jaringan lemak, otot dan lainnya terjebak dalam keretakan tersebut, sehingga mengakibatkan penglihatan ganda. Penyakit terkait dalam orbit seperti tumor dan infeksi juga dapat mengakibatkan bola mata bergeser dari orbit, sehingga menyebabkan gejala diplopia.

Background Diplopia is the subjective complaint of seeing 2 images instead of one and is often referred to as double-vision in lay parlance. The term diplopia is derived from 2 Greek words: diplous, meaning double, and ops, meaning eye. Diplopia (double vision) is a common subjective complaint, or diplopia may be elicited during the course of an eye examination. Diplopia is often the first manifestation of many systemic disorders, especially muscular or neurologic processes .[1] An accurate, clear description of the symptoms (eg, constant or intermittent; variable or unchanging; at near or at far; with one eye [monocular] or with both eyes [binocular]; horizontal, vertical, or oblique) is critical to appropriate diagnosis and management.[2, 3] Binocular diplopia can be corrected by covering either eye; monocular diplopia persists in one eye despite covering the other eye. Physiologic diplopia is a normal phenomenon depending on the alignment of the ocular axes with the objects of regard (eg, focusing on a finger held close results in distant objects being blurry but double). Polyplopia refers to the perception of more than 2 images and is often a monocular phenomenon caused by refractive aberrations resulting in multiple images of one object. In such cases, the dominant image of the object of regard is accompanied by secondary images that may be less intense, distorted, or fleeting. Causes of polyplopia include irregular corneal astigmatism, lenticular opacities, multifocal lenses, and corneal rings of significantly different focality within the pupil created by refractive surgery or contact lenses.
Animal models Unless the visual fields of the eyes overlap, binocular diplopia cannot occur. Among vertebrates, the potential for diplopia (and for stereoscopic depth perception) depends on where the eyes are located in the head. Eyes located on either side of the head provide a wide visual field but with a less overlapped visual field. These animals have less field for binocular vision and less risk for diplopia when one eye becomes misaligned. However, when both eyes are located in the front of the head, a greater visual field overlap exists and, thus, a better binocular depth perception, as frequently seen in predators. Misalignment of such eyes may result in diplopia. Monocular diplopia is often due to optical aberrations resulting in multiple images. The eyes of birds demonstrate many unique anatomical features, one of which is the presence of multiple foveae and, in some cases, a streak fovea linking 2 foveae. Thus, they may be able to have 2 separate areas of regard without disabling diplopia. How the visual perception occurs in these cases remains debatable.
Pathophysiology Binocular diplopia (or true diplopia) is a breakdown in the fusional capacity of the binocular system. The normal neuromuscular coordination cannot maintain correspondence of the visual objects on the retinas of the 2 eyes. Rarely, fusion cannot occur because of dissimilar image size, which can occur after changes in the optical function of the eye following refractive surgery (eg, LASIK) or after acataract is replaced by an intraocular lens. The distortion of one image may be interpreted as diplopia by the patient; however, the same object does not appear to be in 2 places but rather appears differently with each eye. Monocular diplopia may occur from abnormal ocular media (eg, corneal distortion or scarring, multiple openings in the iris, cataract or subluxation of the natural lens or pseudophakic lens implant, vitreous abnormalities, retinal conditions). Monocular diplopia must be distinguished from metamorphopsia, in which objects appear misshapen.
Epidemiology Frequency United States
No figures are available as to prevalence of diplopia in the United States.
International
International incidence rates of diplopia are unknown. The incidence of diplopia as a chief complaint in emergency departments is low. One study of a specialist eye hospital in London, United Kingdome reported the incidence of diplopia as the chief complaint in only 1.4% of the presenting cases . [4]
Mortality/Morbidity Divergent pathological processes, each with its own morbidity and mortality, can cause diplopia. However, irrespective of cause, diplopia has significant morbidity in terms of diff iculty with depth perception and confusion with orientation of objects, especially when performing visually demanding tasks, such as driving a vehicle or operating tools. Therefore, in assessing visual disability after injuries, loss of binocularity accounts for a major percentage of loss of function.
Race No information is available regarding differences in various racial groups.
Sex No information is available suggesting differences in prevalence with respect to sex.
Age Diplopia is encountered almost exclusively in adults or in those with mature visual systems because of the following:
Young children may not be able to express this symptom. More importantly, the immature visual system deals with diplopia by suppressing the poorer image, possibly resulting in irreversible amblyopia. Children with obvious and marked ocular malalignment from strabismus are comfortable and content because the visual image from the deviating eye is suppressed and not noticed. In contrast, adults who have mature visual processing pathways cannot easily ignore the second image. Wearing a patch over one eye to prevent visual confusion usually is the only option for these patients.

AAA Double vision, or diplopia, is a symptom to take seriously. Some causes of diplopia are relatively minor, but others need urgent medical attention. WebMD takes a look at the causes, symptoms, and treatments of double vision. What Causes Double Vision? Opening your eyes and seeing a single, clear image is something you probably take for granted. But that seemingly automatic process depends on the orchestration of multiple areas of the vision system. They all need to work together seamlessly:
The cornea is the clear window into the eye. It does most of the focusing of incoming light. The lens is behind the pupil. It also helps focus light onto the retina. Muscles of the eye -- extraocular muscles -- rotate the eye. Nerves carry visual information from the eyes to the brain. The brain is where several areas process visual information from the eyes. Problems with any part of the vision system can lead to double vision. Cornea problems. Problems with the cornea often cause double vision in one eye only. Covering the affected eye makes the double vision go away. The abnormal surface of the eye distorts incoming light, causing double vision. Damage can happen in several ways:
Infections of the cornea, such as herpes zoster , or shingles, can distort the cornea. Corneal scars can alter the cornea, creating unequal visual images. Dryness of the cornea can create double vision. Lens problems. Cataracts are the most common problem with the lens that causes double vision. If cataracts are present in both eyes, images from both eyes will be distorted. Cataracts are often correctable with minor surgery. Muscle problems. If a muscle in one eye is weak, that eye can't move smoothly with the healthy eye. Gazing in directions controlled by the weak muscle causes double vision. Muscle problem s can result from several causes:
Myasthenia gravis is an autoimmune illness that blocks the stimulation of muscles by nerves inside the head. The earliest signs are often double vision and drooping eyelids, or ptosis. Graves' disease is a thyroid condition that affects the muscles of the eyes. Graves' disease commonly causes vertical diplopia. With vertical diplopia, one image is on top of the other. Nerve problems. Several different conditions can damage the nerves that control eye muscles and lead to double vision:
Multiple sclerosis can affect nerves anywhere in the brain or spinal cord. If the nerves controlling the eyes are damaged, double vision can result. Guillain-Barre syndrome is a nerve condition that causes progressive weakness. Sometimes, the first symptoms occur in the eyes and cause double vision. Diabetes can lead to nerve damage affecting the muscles which move the eyes, causing double vision.
Brain problems. The nerves controlling the eyes connect directly to the brain. Further visual processing takes place inside the brain. Many different causes for double vision originate in the brain. They include:
Strokes Aneurysms Increased pressure inside the brain from trauma, bleeding, or infection Brain tumors Migraine headaches
Double Vision (Diplopia) What Are the Symptoms of Double Vision? Double vision can occur by itself with no other symptoms. Depending on the cause, other symptoms may be present with double vision, such as:
Misalignment of one or both eyes (a "wandering eye" or "cross-eyed" appearance) Pain with eye movements in one or both eyes Pain around the eyes, such as in the temples or eyebrows Headache Nausea Weakness in the eyes or anywhere else Droopy eyelids How Is Double Vision Diagnosed? Double vision that's new or unexplained needs urgent medical attention. With so many potentially serious causes for double vision, it's important to discover the reason without delay. Your doctor will most likely use multiple methods to diagnose the cause for double vision. Blood tests, a physical exam, and possibly imaging studies like computed tomography (CT) or magnetic resonance imaging (MRI) are frequently used. One of the most effective tools in diagnosing diplopia, though, is the information you can provide. You can make the diagnosis for double vision more accurate by answering several questions beforehand.
When did the double vision start? Have you hit your head, fallen, or been unconscious? Were you in a car accident? Is the double vision worse at the end of the day or when you're tired? Have you had any other symptoms besides double vision? Do you tend to tilt your head to one side? Look at old pictures, or ask family -- you may not even be aware of the habit. Now, focus on something unmoving in your field of vision -- a window or a tree.
Are the two objects side by side, or is one on top of the other? Or are they slightly diagonal? Which one is higher or lower? Are both images clear but simply unaligned with each other? Or is one image blurry and the other clear? Cover one eye, then uncover it and cover the other. Does covering either eye make the double vision go away?
Pretend your field of vision is a clock face. Move your eyes around the clock, from noon to six and up to 12 again. Is your double vision worse at any clock position? Does any position make your double vision improve? Tilt your head to the right, then to the left. Do any of these positions improve the double vision, or make it worse? How Is Double Vision Treated? With double vision, the most important step is to identify and treat the underlying cause. In some cases, double vision can be improved by managing or correcting its cause.
If weak eye muscles are the cause, or if a muscle has been pinched as a result of injury, surgery may help. Myasthenia gravis can be treated with medications. Graves' disease is treatable with surgery or medical therapy. Blood sugar in diabetes can be controlled with medicines and/or insulin. If double vision can't be reversed, treatments can help people live with double vision. Sometimes, this requires wearing an eye patch or special prism glasses to minimize the effect of double vision.

Diplopia Monokular: penglihatan ganda muncul saat salah satu mata ditutup atau terjadi pada satu mata. Biasa terjadi pada penderita astigmatisma, gangguan lengkung kornea, penderita katarak, dislokasi lensa mata, gangguan produksi air mata dan beberapa gangguan pada retina. Pengobatan diplopia tergantung dari penyebab dasar yang menjadi pemicunya. Upaya pertama yang bisa dilakukan antara lain penggunaan kacamata sesuai ukuran, operasi pembedahan, dan terapi tanpa operasi yang berfungsi menyelaraskan mata serta merangsang jalur penglihatan ke otak yang tidak bekerja dengan baik. Sebelum membahas lebih jauh Diplopia, mari kita harus mengerti Apa itu diplopia? Diplopia atau penglihatan ganda adalah suatu gangguan penglihatan yang mana obyek terlihat dobel atau ganda. Diplopia berasal dari bahasa Yunani, diplo = dobel atau ganda, opia = penglihatan. penyakit ini, ternyata ada berbagai jenis diplopia. Ada Diplopia binokular, monokular, vertical, horizontal, dan ada beberapa lagi. Diplopia binokular, yaitu penglihatan ganda terjadi apabila si pasien melihat dengan kedua mata dan menghilang bila salah satu mata ditutup. Diplopia monokular yaitu diplopia yang hanya terjadi pada satu mata. Penglihatan ganda muncul saat salah satu mata ditutup. Sedangkan vertikal atau horizontalnya adalah posisi bayangannya. Diplopia adalah bersifat temporary atau sementara, ada juga yang bersifat permanen. Semuanya tergantung dari penyebabnya, biasanya efek dari penyakit lain, seperti astigmatisme (silindris), gangguan lengkung kornea, pterigium, katarak, dislokasi lensa mata, gangguan produksi air mata, dan beberapa gangguan pada retina, atau bisa juga karna syaraf mata yang kelelahan, kelelahan ini biasanya yg dialami oleh orang2 yang bekerja dengan melihat objek yang terlalu dekat terus menerus, seperti di depan komputer, baca buku, atau maen handphone terus menerus. Jadi tidak selamanya mata berbayang ini karna penyakit mata s ilindris, yang bisa diatasi dengnan kacamata atau lensa kontak. Meskipun faktor penyebab dan gejalanya sama. bisa jadi ini disebabkan penyakit lain atau gangguan syaraf mata lainnya. Dan juga meskipun tidak ada penyakit lain yang parah yg menyebabkan atau hanya karna kecapean, Diplopia Temprorary juga butuh waktu yang tidak sebentar untuk menyembuhkannya. Bagi yang suka seharian depan komputer, atau yg sering baca buku berjam-jam, ser ing2lah melihat objek jauh atau pemandangan landscape ke jendela selama 15 menit setiap 2 jam. begitulah kata dokter specialis mata.
